Spravato® is an FDA-approved prescription treatment for certain adults with depression and is administered under the supervision of a healthcare provider in a certified treatment setting.

What Is Spravato®?
Spravato® contains esketamine, a prescription medication that works differently from many traditional oral antidepressants. It is a nasal spray that acts on the brain’s glutamate system through NMDA receptors.
Spravato® is indicated for:
- Treatment-resistant depression (TRD) in adults, either as monotherapy or in conjunction with an oral antidepressant.
- Depressive symptoms in adults with major depressive disorder (MDD) with acute suicidal ideation or behavior, in conjunction with an oral antidepressant.
Spravato® is administered only under the supervision of a healthcare provider and is available through the SPRAVATO® REMS (Risk Evaluation and Mitigation Strategy) program.
Who May Be a Candidate for Spravato®?
Spravato® may be considered for adults whose depression has not responded adequately to previous antidepressant treatment or for adults with MDD experiencing acute suicidal ideation or behavior.
Before treatment, our providers will complete an evaluation of your symptoms, medical history, current medications, and treatment history to determine whether Spravato® is an appropriate option.
What to Expect During Treatment
Spravato® treatment takes place in a supervised healthcare setting.
Before Treatment
Your provider will:
- Review your medical and psychiatric history.
- Discuss your previous treatment response.
- Review your current medications and overall health.
- Determine whether Spravato® is appropriate for you.
- Explain what to expect during and after treatment.
During Treatment
Spravato® is administered as a nasal spray under the direct supervision of a healthcare provider. Because the medication can cause effects such as sedation, dissociation, and changes in blood pressure, you will be monitored throughout the treatment session.
After Treatment
You will be monitored for at least two hours following administration. Your provider will assess your condition and determine when you are clinically stable and ready to leave.
Because Spravato® can temporarily impair alertness and coordination, you will need to arrange transportation home and should not drive or operate machinery until the next day following a restful sleep.
Important Safety Information
Spravato® has important risks and is available only through the SPRAVATO® REMS program.
Treatment may cause sedation, dissociation, respiratory depression, increased blood pressure, dizziness, nausea, and other side effects. Spravato® also has the potential for abuse and misuse because it contains esketamine, a Schedule III controlled substance.
Spravato® is contraindicated in certain patients, including those with:
- Aneurysmal vascular disease or arteriovenous malformation
- A history of intracerebral hemorrhage
- Hypersensitivity to esketamine, ketamine, or certain ingredients in the medication
Spravato® is not approved for pediatric patients. It is also not recommended during pregnancy, and breastfeeding is not recommended during treatment.
Limitations of Use
The effectiveness of Spravato® in preventing suicide or reducing suicidal ideation or behavior has not been demonstrated. Treatment with Spravato® does not eliminate the need for hospitalization when clinically warranted.
Spravato® is not approved as an anesthetic agent, and its safety and effectiveness as an anesthetic have not been established.
